Blonanserin was developed as an antipsychotic drug in Japan and is approved for the treatment of schizophrenia. It belongs to a series of 4-phenyl-2-(1-piperazinyl)pyridines.

II. Mechanism of Action
Blonanserin acts as an antagonist at dopamine D2, D3, and serotonin 5-HT2A receptors. It binds to and inhibits these receptors with high affinity. It has low affinity for other dopamine and serotonin receptors as well as muscarinic, adrenergic, and histamine receptors. This medicine regulates neurotransmitters (e.g., dopamine, serotonin) in the brain to improve symptoms such as strong anxiety, tense feeling, and lowering of motivation.

III. Characteristics
Blonanserin is a dopamine-serotonin antagonist with a high affinity to D2,3 and 5HT2A receptors. Its affinity to muscarinic, histaminic, and adrenergic receptors is very low, leading to a reduced risk of weight gain, abnormal lipid or glucose metabolism, and cardiovascular complications.

IV. Uses in the Treatment of Mental Disorders

Schizophrenia: It is mainly used to treat schizophrenia to manage symptoms such as hallucinations, delusions, disorganized thinking, and social withdrawal.

Other Psychotic Disorders: May also be used in the treatment of other psychotic disorders where there are disturbances in thought, perception, and behavior.

V. Managing Symptoms
Helps in reducing positive symptoms like hallucinations and delusions as well as negative symptoms such as apathy, social withdrawal, and lack of motivation. Can improve cognitive function and overall functioning in patients with psychotic disorders.
